Chiropractic Treatment of Lower Back Spasms
Many people have lower back pain and sciatica from poor posture and tight muscles. This can happen little by little and day after day from repetitive activities. It can also happen suddenly while aggressively performing activities you have not done in a while or when you over do it, by lifting something wrong or too heavy.
If you have muscle spasms, here is what you can try or need to be aware of to get relief:
Rest: Do not put any unnecessary stress on the muscles, get in the most comfortable position. Pain is a great teacher of what does and does not work.
Water: Dehydration can cause muscle spasms. Make sure you are drinking water, that is the oil that lubricates the muscles.
Electrolytes: Being low on electrolytes can cause muscle spasms. Electrolytes help your cells retain water.
Ice: I always suggest starting with ice, 15 minutes on, 40 minutes off, as many rounds as you would like. In the beginning try for at least 6 rounds a day. This works great for most people. For more chronic issues, after you have tried ice, some people find heat makes their muscle spasms feel better. Some people try alternating, if you do, always end with ice. Caution, heat can feel relaxing, but as your back cools it can start to spasm more. This would be another indication that ice is for you.
Stretching: We do not suggest stretching or pulling on muscles that are in spasm. Muscle in spasm do not relax and lengthen, they usually tighten when they are stretched. However, once the spasms subside and are reduced to tight and stiff muscles, then we are big fans of responsibly introducing basic and very light stretches as a chiropractic treatment for lower back pain and sciatica.
Medication List: Check your medication list with your pharmacist. There are many medicines that have cramping as a side effect.
Kidney Stones: See you doctor if you have a history of kidney stones or feel kidney stones may be the cause of your lower back pain.
If you do not get relief from following these steps, this would be when you would want a chiropractor for an in-office evaluation and treatment. They will take the time to make a wise diagnosis based on all the different factors that may be involved and provide care to restore health and relief to the affected area.
If you have tried reducing muscle spasms and have not found relief, it may be the lower back that is causing the pain, muscle spasms, and even sciatica.
Here are common causes of lower back pain and sciatica:
Poor Posture, Lack of Flexibility, Weak Core, Never Learning how to Keep Your Spine Healthy: These are basically the root cause of all spine, lower back, and sciatica problems.
The most important being "how to keep your spine healthy. You can improve posture, improve flexibility, and strengthen your core in weeks, however if you let your spine go, you can't get it back to where it was years ago, you have to start with the one you have right now and go from there. Many people are inactive and living with pain because they have never learned chiropractic treatments to help them keep their spine healthy as they aged.
Here is how lower back problems lead to muscle spasm, lower back pain, and sciatica:
Stuck Joints: when joints aren't moving because of repetitive stress or trauma, they are surrounded in inflammation. This inflammation irritation joints, discs, muscles and nerves.
Arthritis: If joints remain stuck for days, weeks, months, and years they age faster and become arthritic. These joints in untreated can cause more irritation to muscles and nerves, while producing more inflammation and irritation to the spine and discs.
Disc Degeneration: Just like with arthritis, stuck joints cause the discs of the spine to age and degenerate faster. This can also contribute to irritation to muscles and nerves.
Here are chiropractic treatments for lower back pain and sciatica:
Ice: Ice is the best place to start, 15 minutes on, 40 minutes off, at least 6 rounds if you are in pain.
Stretching, Foam Rollers, Massage, Muscle Therapy: Always start slow! If the pain is not too bad and does not aggravate your condition, improving muscle health with these techniques will always play a major role in achieving faster, long lasting relief, while improving the health of your muscles. When it comes to sciatica the muscle you will want to focus on is the piriformis muscle. If the piriformis muscle is contributing to your sciatica, learning how to stretch and foam roller this muscle, in time, will produce great relief.
Strengthening: If you have gotten relief from ice and muscle therapy, then the next step would be to strengthen your core, so your muscles are healthier and stronger. This will provide your muscles what they require to do a better job at protecting your spine.
Chiropractic Adjustment: A good history, examination, x-rays (if necessary) with a chiropractor, is the fastest way to learn what is causing your muscle spasms, lower back pain, or sciatica. Chiropractic care has an amazing track record over the last 120+ years for being the safest, fastest, longest lasting form of relief for headaches, neck, back, lower pain pain, and sciatica.

If you suffer from low back pain, sciatica, hip pain, or knee pain, the shoes you wear can significantly impact your comfort and mobility. Many people overlook the importance of shoe drop and stack height when selecting footwear, but these factors play a critical role in spinal alignment and joint health.
At Fresh Start Chiropractic in Linwood, NJ, Drs. Jennifer and John Fresh, with over 20 years of experience, help patients understand how footwear affects their posture, movement, and overall spinal health. Let’s dive into why choosing the right shoes is essential and how a systematic approach to testing different shoes can help you find the perfect fit for pain relief and performance.
Before discussing their effects on the spine, let’s define these two important terms:
• Shoe Drop (Heel-to-Toe Drop) – This is the difference in height between the heel and forefoot of the shoe. A higher drop (over 8mm) means the heel sits much higher than the forefoot, while a lower drop (0-4mm) keeps the foot more level.
• Stack Height – This refers to the thickness of the shoe’s midsole. A higher stack height provides more cushioning, while a lower stack height allows for more ground feel.
A higher shoe drop shifts the body’s weight forward, encouraging more low back extension (arching). While this may feel comfortable initially, it can be problematic for people with conditions such as:
• Facet Syndrome – Excessive low back extension compresses the facet joints, increasing irritation and pain.
• Decreased Intervertebral Foramen (IVF) Space – This is the small opening where nerves exit the spine. Excessive arching narrows this space, leading to nerve compression, irritation, and potentially sciatica.
• Anterolisthesis (Forward Vertebral Slippage) – If you have instability in your spine, high-heel drops can worsen forward slippage of the vertebra, leading to increased pain and dysfunction.
For individuals with these conditions, a lower shoe drop (0-4mm) may be better suited to keep the spine in a more neutral position and reduce stress on the low back.
Your body is designed to absorb and dissipate stress efficiently, but this only happens when your joints and muscles are healthy. At Fresh Start Chiropractic in Linwood, NJ, we emphasize that proper spinal alignment and muscle flexibility are key to protecting your joints from excessive wear and tear.
• Even Distribution of Stress – A balanced and mobile body spreads forces evenly across the feet, ankles, knees, hips, and spine.
• Reduced Impact on Joints – A properly functioning musculoskeletal system reduces the likelihood of joint damage, arthritis, and chronic pain.
• Improved Endurance and Recovery – When stress is absorbed correctly, your body can cover more miles without excessive fatigue or injury.
When joints are stiff or misaligned and muscles are tight, the body loses its ability to absorb shock effectively. Instead of being evenly distributed, stress gets transferred into specific areas, leading to:
• Increased pressure on the feet, ankles, knees, and hips
• More strain on the low back
• Faster joint wear and tear, contributing to conditions like arthritis
If you’re experiencing low back pain, sciatica, hip, knee, or leg pain, chiropractic care can help restore alignment and flexibility, allowing your body to handle stress more efficiently.
For some people, a higher stack height (more cushioning) can help absorb impact forces from walking, running, or hiking. This is especially beneficial for:
• Individuals with joint pain or arthritis who need extra shock absorption
• People who spend long hours walking on hard surfaces
• Runners or hikers who experience foot fatigue and soreness
However, too much cushioning can reduce ground feel and stability, which can be problematic for some. The key is finding the right balance between cushioning and responsiveness for your specific needs.
When looking for the best shoes to support your low back, hips, and knees, it’s important to take a systematic approach rather than relying on marketing claims. Here’s how:
1. Identify Your Needs
• Do you have low back pain, sciatica, hip pain, or knee pain?
• Do you have facet syndrome, decreased IVF space, or anterolisthesis?
• Are you looking for shoes for walking, running, or everyday wear?
2. Test Different Shoe Drops
• If you have low back pain due to excessive extension, try a lower drop (0-4mm).
• If you have tight calves or Achilles issues, a moderate drop (4-8mm) might be better.
3. Experiment with Stack Heights
• If you need more shock absorption, try a higher stack height.
• If you prefer more ground feel and stability, go for a lower stack height.
4. Pay Attention to Comfort and Recovery
• Do you feel less pain and fatigue after wearing certain shoes?
• How well do you recover after a long walk, hike, or run?
• Do certain shoes allow you to be active longer without discomfort?
5. Make Changes Gradually
• For best results make all changes to your current heel drop and stack height gradually
• If possible change only shoe drop or stack height at a time to determine, which factor has the greatest impact.
• In the end it may be a combination of both. That’s ok, because you will know which shoe is best for you because you got to experience each variation. You may be wearing this style shoe for a long time so it is worth the effort to find the best fit for you.
• Don’t be fooled by just what feels better, when you can find what feels the best and is the best for your spine and joints.
• If your body goes through any changes over time, what works today might be a little different tomorrow.
• Shoes are very important, every so often it is worth it to give them some consideration and be proactive about any changes you may need to make.
